Format B — For the Nursing Team: Numbered discharge checklist: medications administered, education provided, next appointment confirmed, warning signs reviewed, transportation arranged.
Format C — For Maria (patient-facing): Plain-language letter in two columns: ‘Your Diagnosis’ | ‘What It Means For You’ Include: warning signs to watch for, medications to take at home, when to call the doctor.
